Is the streaming server only for content saved to file or are there options for realtime streaming say from a video source with low lag capability? Media Encoder has always been a poor option for realtime streaming due to the awful lag and I was wondering if Silverlight might provide a better option.

Then the Silverlight Streaming service offers the former, i.e. streaming of content saved to file, on-demand.

To the mods on here : I think we're having these topics spread out a bit between "Hosting and Streaming" and "Video and Media". Maybe you can clarify what the best place is to discuss Silverlight's support for Windows Media streaming.
To 23501a : Silverlight supports both live and on demand video. On demand video you can either put on any webserver and have it downloaded progressively by SIlverlight, or you can host it with a Windows Media Server and have it streamed. Live streams you can host with Windows Media Server. Check this other thread for more info about low latency streams http://silverlight.net/forums/t/440.aspx .
